To schedule an exam, please call us at (916) 779‑1018. If the exam was requested by your physician, please have your referral request and your insurance information available when you call. Before you arrive for your exam, be sure to download and fill out the New Patient Form at right.

Insurance, Co-Payments and Deductibles
Please remember to bring your insurance card and co-payment or deductible as well as a photo ID. Payment is expected at the time of your visit. We accept cash, VISA and MasterCard. For a list of insurance plans we accept, please click here.
Arriving for Your Exam
Traffic: Allow extra time during rush hours (7 to 9am and 4 to 7pm)
Arrive early: please plan to arrive 15 minutes before your scheduled appointment time to fill out any additional paperwork that may be necessary. If you are late, chances are that we will have to re-schedule your appointment.
48-hour Cancellation Notice is Required
As we only serve one client at a time, an appointment means that we reserved that specific time for you. If you need to cancel or reschedule your appointment, please call us at least 48 hours in advance.
